**Description** As a member of the interdisciplinary team, the Rehabilitation Department Supervisor oversees and coordinates all rehabilitation services provided by Sutter SeniorCare, including physical therapy, occupational therapy and speech therapy. Responsibilities include maintaining adequate staffing; staff supervision of ADHC and off-site service; overseeing coordination and utilization of assistive devices, rehabilitation and durable medical equipment; primary liaison/consultant to other Sutter SeniorCare departments and outside entities to promote safe and effective coordinated services; and provision of rehabilitation services within established budgetary guidelines. **SUPERVISES REHABILITATION DEPARTMENT STAFF:** + Recruits, hires, trains, supervise, and evaluate department staff. + Ensures appropriate staffing levels for patient care within established budgetary guidelines. + Supervises and coordinates rehabilitation consultants as necessary. **PROVIDES PROGRAM ADMINISTRATION:** + Develops policies and procedures for all rehabilitation services including physical therapy, occupational therapy and speech therapy. + Develops and supervises participant evaluation, home assessments and treatment priorities based on input from the staff and other Sutter SeniorCare entities. + Communicates routinely with Department staff including regular Department staff meetings to discuss participant and department issues. + Assists the Rehabilitation Department specialists, e.g. orthotists, prosthetists, durable medical equipment vendors to assure maximum benefit to the participants at reasonable costs. + In conjunction with Rehabilitation Department and other Sutter SeniorCare staff, plans and carries out quality improvement studies quarterly. **MANAGE DURABLE MEDICAL EQUIPMENT (DME) AND ASSISTIVE DEVICES:** + Develops and manages a system for ordering, inventorying, issuing, tracking and servicing durable medical equipment and assistive devices for participants at home and at the ADHC. + Coordinates services and repair of DME and assistive devices on a routine basis and as needed. + Coordinates, assesses, and orders durable medical equipment and adaptive devices for participants and for use in the ADHC within the established budgetary guidelines. **ACTS AS A CONSULTANT AND EDUCATOR REGARDING ALL REHABILIATION SERVICES AND PROCEDURES TO INSURE HIGH QUALITY, EFFECTIVE DELIVERY OF CARE, AND MAXIMUM SAFETY OF PARTICIPANTS, THEIR FAMILIES AND STAFF IN THE ADHC AND AT HOME.** + Makes visits/educates other facilities in regard to rehabilitation services and participant care as needed. + Instructs and demonstrates to family and other caregivers the rehabilitative procedures indicated for the individual participants. + Assesses the participant’s home environment and makes recommendations for adaptations, which will increase independence and/or enhance safety. + Together with the staff physician, home health/home care nurse, physical and occupational therapists plans for and orders equipment and supplies needed by the participant to enhance mobility, independent function and safety. **LIAISON WITH OTHER STUTTER SENIORCARE DEPARTMENTS AND OUTSIDE ENTITIES:** + Participates in team meetings, supervisor’s meetings, quality assurance teams and other Sutter SeniorCare activities as appropriate. + Represents Sutter SeniorCare or the Rehabilitation Department to outside entities as appropriate. + Assists Accounting Office Manager in identifying vendors most appropriate to contract with for services. **Provides Direct Patient Care** + Completes post-intervention activities: session closure, documentation, communication, charge capture. + Prepares for therapeutic intervention. + Provides initial evaluation of patient's abilities and designs appropriate intervention plans. + Treats patients with therapeutic procedures, modalities and or equipment to improve function **Qualifications** **Education / Licensure / Certification / Registration:** + Bachelor’s degree in physical or occupational therapy.
