Clinical Care Coordinator, Sutter Senior Care (Full time-Days)
Develops and coordinates care environment and total nursing care for patients. Monitors trends in the delivery of patient care practices, evaluates clinical performance, and provides nursing practice leadership in quality Improvement processes. Works collaboratively with physicians and other interdisciplinary team personnel to maintain standards for professional practice in the clinical setting. Effectively provides age appropriate care. Important dimensions of this position consist of Clinical Judgment and Quality of Care; Caring Practices; Ethical Practice and Response to Diversity; Systems Effectiveness; Clinical Inquiry and Learning; and Leadership and Collaboration. Meets the performance expectations of the Staff Nurse II role. These dimensions incorporate the competencies defined by the California Board of Registered Nursing and the ANA Standards of Clinical Nursing Practice.
The Clinical Care Coordinator in conjunction with the Clinical Manager, Primary Care Clinic/Adult Day Health Center is responsible for coordinating, providing and supervising nursing care for participants. This position is responsible for performing triage, in the Primary Care Clinic/ADHC, planning and coordinating discharge or transfer of participants from inpatient settings, Skilled nursing facility, Residential Care facility, board and care facility, or home, conducting initial and periodic re-assessment of participant’s nursing care needs. The Clinical Care Coordinator is a member of the Interdisciplinary team, which coordinates and develops participant’s comprehensive care plan. The person in position can act as Clinical Manager when the Clinic Manager is on vacation or other leave.
+ Graduate of an accredited school of nursing. BSN preferred.
**Licensures and Certifications**
+ Currently licensed as a Registered Nursing in State of California.
+ Effectiveness in leading clinical teams, communication, and interdisciplinary collaboration required.
+ Experience in acute care or sub-acute care nursing, ambulatory care setting or urgent care required.
+ Must have experience caring for the frail elderly in a community based or institutional setting.
+ Knowledge of the interdisciplinary model of care planning and experience in working with seniors who are cognitively impaired is important.
**Skills and Knowledge**
+ Effectiveness in leading clinical teams, communication, and interdisciplinary collaboration.
+ Experience in acute care, sub-acute care nursing.
+ Demonstrated knowledge of aging process and its influence on health and functional status of elderly.
+ Must be able to assess health status, including triaging, of the frail elderly.
+ Knowledge of all State and Federal laws, regulations and other requirements for Sutter SeniorCare/PACE that is applicable to this position.
+ Demonstrate effective English verbal and written communication skills.
+ Demonstrated excellent organizational skills. Demonstrated ability to multitask.
+ Must have excellent interpersonal and time management skills.
+ Effective problem-solving and conflict resolution skills.
+ Strong assessment and critical thinking skills necessary to determine services appropriate for participants with complex medical, emotional and social needs.
+ Must be able to work effectively in the Interdisciplinary Team Model.
+ Must be able to work independently with minimal supervision.
+ Must be able to deal well with emergency situations and demonstrate flexibility with changes in daily routines.
+ Basic computer skills.
+ Ability to independently organize and prioritize workload with strong attention to detail.
+ Demonstrate ability to establish positive working relationships with staff.
+ Prioritizes and manages multiple demands in an ever changing environment.
