**Description** The Stroke Coordinator is a registered nurse (RN), responsible for the coordination and management of the stroke program and oversees the care of stroke patients at Sutter Medical Center Sacramento. The Stroke Coordinator will work collaboratively with the Medical Director of the Stroke Program and the Regional Neuroscience Institute to ensure the care for the stroke patient throughout the continuum. The Stroke Coordinator collaborates with the stroke leadership team members, to ensure comprehensive clinical treatment, decisions, and diagnoses for inpatients and will oversee medical record documentation to reflect best practice standards of care. The Stroke Coordinator will build relationships, support patients, and solve problems with the end goals being elimination of barriers to care, timely delivery of services, and better patient outcomes. Deliverables of the Stroke Coordinator include: drive quality outcomes in stroke care through successful implementation and monitoring of current evidence based clinical practice guidelines. Coordinates and ensures effective communication and documentation across the continuum of care; between physician specialists, internists, the breadth of clinical and non-clinical support service providers and the patients and their families. Provides input to patient assessment processes and treatment care plans for the patient population and suggests/drafts revisions to or devises new policies and procedures that guide and support the provision of services. Maintains constructive collegial relations and collaborates with other clinical/support service managers in cross-functional process improvement initiatives, facilitates open communication and problem solving to ensure the effective coordination and integration of services within and between units. Assures that SMCS maintains a patient-centric focus by serving as a role model with consistent emphasis on patient and family relations, providing feedback regarding positive and negative customer comments and participating in customer satisfaction and clinical outcomes initiatives. Continuously invests in personal and professional growth, education and development and remains current on all matters relating to managing the Stroke Program. Collaborates with hospital leadership on all relevant employee mandatory competencies, training, on dedicated neuro inpatient units and ED. Quality/Stroke Survey Prep: Identifies, implements and coordinates stroke survey preparation including but not limited to: data collection and monitoring of key performance measures; staff competencies and continuing stroke education as well as EBP protocol/ guidelines oversight. Supervises the direction and compliance of policy/procedure/protocol development, committee activities and projects as they pertain to the development of the Stroke Program. Responsible for directing all Stroke Program meetings and if necessary arranges for high level meetings between departments to ensure that the program runs effectively. Collects data with the objective of analyzing for quality measures in a timely fashion, addresses outcomes in stroke patient population and opportunities for performance improvement Supervises the activities and direction of the Stroke Program staff. **Qualifications** **Requirements:** + Valid CA RN License Required + Certified Neuroscience RN - CN-Neuro is required. (If not current, obtained within one year of hire) + Basic Life Support (BLS) Certification (for the healthcare provider) is required. + ACLS is required + Completion of Human Research Subject Certification (ex: CITI) is required within 6 months of hire. **Education:** Bachelor's degree in Nursing is required. Master's degree in Nursing is preferred. **Experience:** + Minimum of 3 years clinical experience in the care of stroke patients in the inpatient setting with a minimum of one year experience in critical care of ER preferred. + Three years’ experience in Performance Improvement strategies, comprehensive knowledge of regulatory, accrediting and Primary Stroke Center designation requirements iss required. + Experience with all aspects of clinical research and trials preferred. + Previous experience working in case management preferred. + Experience with Comprehensive Stroke Center accreditation process preferred **Skills and Knowledge:** + Must demonstrate excellent writing skills and knowledge of correct business English; strong organization skills, ability to perform multiple tasks with interruptions and manage time effectively to achieve simultaneous goals + Must demonstrate excellent interpersonal skills to effectively communicate with physicians, hospital personnel and external agencies, both verbally and in writing.
