Under general supervision, the Mental Health Worker - Discharge Planner performs a wide variety of duties to assist professional mental health services staff in coordinating patient discharge for individuals with an acute or persistent mental health disorder or co-occurring disorders.
Minimum Qualifications:
Either: 1. One year of full time, paid experience in the class of Mental Health Worker in Sacramento County Service.
Or: 2a. Possession of an Associate's Degree, or higher from an accredited college or university in a mental health related field such as Social Work, Rehabilitation Counseling, Psychology or Counseling; or 60 semester units (90 quarter units), from an accredited college or university, of which 24 semester units (36 quarter units) must have been in Social Work, Rehabilitation Counseling, Psychology or Counseling.
2b. One year of full time, paid experience performing care coordination, case management, and/or discharge planning in a local, state, or federal agency and/or in a clinical setting.
Note Applicable to Pattern 2: Additional experience as described in section 2b may substitute for the required education on a year for year basis. (1 month of experience is equal to 2.5 semester (3.75 quarter) units)
