Under the direction and supervision of the Director of Home Health/Home Care, the Home Health/Home Care LVN provides skilled nursing services in the home to Sutter SeniorCare Participants.
**Education:** Valid and current California Licensed Vocational Nurses License.
**Experience:** One year LVN experience in Hospital, Home Health or Long Term Care. Home-care experience desirable with emphasis on working with the frail elderly. Experience in overseeing the performance of Personal Care Attendants, and HHA a plus.
**Knowledge:** Knowledge of the caring for adult population. Care of the frail elderly desired. Fluent in reading and writing English. Bilingual is a plus.
+ Must have excellent interpersonal skills, including the ability to build rapport and trust with participants and their families.
+ Must be able to work effectively in the interdisciplinary team model, establishing effective working relationships with all members of the team.
+ Must be able to work independently and be flexible to changes in daily routines.
+ Must have valid California Drivers License and dependable vehicle with Auto Insurance.
+ Must have CPR and First Aid Certificate.
