**Position Overview:** **Job Summary** Participates in the patient/family care and experience through critical thinking, teamwork, effective communication, and nursing knowledge and skills. With the multidisciplinary team setting, is responsible for providing nursing care to the participants in the Primary Care Clinic and Adult Day Health Center under the supervision of the Director of Nursing, ADHC/Clinic. **Principal Accountabilities** Community-Promotes a positive relationship between Sutter Health and the community. Finance - Utilizes resources to deliver quality care in a safe, efficient, and cost-effective manner. Growth - Follows patient/family plan of care to achieve optimal health goals. People - Participates as a member of the health care team in a professional manner according to the Standards of Excellence promoting a positive work environment. Senior Care: Provides functional supervision and education of Personal Care Attendants/Home Health Aides as directed. Quality - Provides optimal care to patients' and family which is collaborative, evidence based and meets national/facility standards to achieve the best possible outcome. Maintains documentation to provide evidence of quality of care. Service - Cares for patient/family according to the Sutter Difference standards and activities ensuring the best patient/family experience. **Qualifications:** **MINIMUM POSITION REQUIREMENTS:** **Education, including Licensure, Certification, Registration** Current California licensure LVN required. BLS-HCP required. Senior Care: First Aid and CPR required within 3 months of hire and maintained thereafter. Graduate of an accredited practical or vocational school of nursing or program. **Knowledge** Must have demonstrated knowledge of basic nursing skills; effective English written and communication skills and basic computer skills. Must have ability to relate well with seniors and their families and with the SeniorCare staff. Must be able to deal effectively with sensitive issues and to problem solve with difficult individual and family situations. Must be flexible in order to deal with emergency situations and schedule changes. **Experience** Must have experience in caring for the frail elderly in a community based or institutional setting.
