**Position Overview:** The After Hours Visit Nurse provides skilled nursing services specific to the needs of the patient, including instruction and evaluation in the home, in alignment with SCAH, mission, goals and objectives, after normal business hours (5:00pm to 8:00am) under physician direction. The After Hours Visit Nurse works with and receives visit instructions from Hospice Partners On-Call. The After Hours Visit Nurse is responsible for obtaining any supplemental physician orders that may be necessary to provide care for the patient. The After Hours Visit Nurse is accountable for ensuring that the plan of care is implemented following professional standards of care and agency specific protocols, and for monitoring and updating the Work Flow process in HCHB. **Qualifications:** **Education / Licensure / Certification** RN graduate from an accredited curriculum - required Bachelor's degree in nursing or health related field - preferred Registered Nurse - State Licensure, RN - required BLS - required Driver License, Valid and in State DL number - required Automobile Insurance - required **_**Additional Requirements_** + Travel; Daily to patient homes, meetings, training, etc. + On-Call / standby; per location rotation guidelines + PM or weekend shift; job is After Hours - 5 pm - 8 am **Experience** Minimum one year of acute medical experience - required Direct patient care experience within the past five years - required Home Health or Hospice experience - preferred **Skills and Knowledge** Must have the ability to function independently and as a member of an interdisciplinary team; ability to exercise independent, sound nursing judgment in planning and providing patient care Exhibits knowledge of patho-physiology and accepted treatment protocols for common hospice diagnoses.
