**Position Overview:** This position is responsible for the oversight of the process of enrollment of new participants into **Sutter SeniorCare (SSC) PACE** and the process of obtaining initial and annual recertification of eligibility from the office of Long Term Care DHCS for individuals interested in enrollment in the Sutter SeniorCare PACE program and Health Plan. This position requires an above-average ability to provide excellent customer service to both internal and external customers. **Principal Accountabilities:** • Oversees all intake and re-certification activities • Manages the multi-disciplinary clinical team intake assessment Level of Care (LOC) certification processes to ensure that all submissions are accurate, timely and complete and meets all state expectations as to content, format and quality • Drive the intake process to coordinate the completion of assessments and certification documentation by the clinical staff and makes adjustments and recommendations as needed to ensure (accountability) all team members are performing at expected levels in completing their section of the document • Optimizes affirmative certification submissions by ensuring the highest quality documentation and supportive data is submitted in a clear and concise manner. Foresees problems and takes corrective actions regarding certifications that are incomplete or substandard • Follows-up with State regulators to determine the cause for deferrals or denials and initiates continuous improvement techniques to ensure that ongoing refinements are made to the process • Interacts with other California PACE providers to gather comparative information and to indentify best practices.
